Partners In Nutrition / Kara Lomen — the CACFP & SFSP claims for every site in discovery
What this is. Every food-program claim located in the government's discovery — the actual state-system (CLiCS) claim records produced from Partners In Nutrition's files, for all 61 sites, across both programs (CACFP At-Risk and the Summer Food Service Program). Each one shows, on its face, who submitted and certified the claim to the Minnesota Department of Education: the sponsor, Partners In Nutrition, under Sponsor ID 2000010186, logged in as Kara Lomen. The Mind Foundry sites (★) are 142 of these claims; the rest are PIN's other sites.
All 487 claims for the 61 sites were entered, submitted, and certified through Partners In Nutrition's sponsor account — under Kara Lomen's login. Each carries the Sponsoring Authority Certification in which the sponsor — not TTA, not Mr. Ibrahim — takes "full responsibility for ensuring that this claim accurately represents the number of meals served" and that "records are available to support this claim." No claim in this discovery was submitted by anyone other than the sponsor.

Provenance & caveats. These are the CLiCS claim records produced in the PIN grand-jury return (Bates GJS-…, secondary stamp PIQC-GJS…), text-extracted and inventoried from the government's discovery. This is the produced universe of claims located to date — every CLiCS claim record in the discovery is a Partners In Nutrition claim; Feeding Our Future's claims system is not part of this production. Each card links to the Bates-stamped PDF exactly as produced, including the sponsor certification page. Defense work product — for counsel review before any external use.
A "claim" is the monthly reimbursement request a sponsor submits to the State for meals served at its sites. Two programs appear here.
CACFP At-Risk claims CACFP
The Child and Adult Care Food Program At-Risk Afterschool meal service — afterschool snacks and suppers. These are the 2021 claims for the Mind Foundry and other PIN sites.
On the form: the meal counts sit in the "At-Risk Afterschool Care Meal Service Information" grid.
SFSP claims SFSP
The Summer Food Service Program (Non-Camp) — summer 2020 breakfasts, lunches, and snacks, before the At-Risk program took over.
On the form: the meal counts sit in the "Meals Information" grid (breakfast / lunch / supper / snacks).
Who is logged in
The header of every record — CACFP and SFSP alike — reads "Kara Lomen · Sponsor: 2000010186 · Partners in Nutrition." The claim is created and submitted from the sponsor's account, the role only a sponsor holds. TTA was never an approved sponsor and had no CLiCS sponsor login.
The Sponsoring Authority Certification
Each claim carries the sponsor's signed attestation:
"I hereby take full responsibility for ensuring that this claim accurately represents the number of meals/milks served by reimbursement category, that records are available to support this claim… I understand that … deliberate misrepresentation may subject me to prosecution under applicable state and federal criminal statutes."
Why it matters: the responsibility for accuracy and supporting records sits with the Sponsoring Authority (PIN), by the State's own form.
Original vs. Adjustment
429 records are Original claims; 58 are Adjustments — claims PIN went back and revised. Adjustments (amber, filterable) show the sponsor actively managing and re-running claims after the fact.
In every record the submitting and certifying party is Partners In Nutrition (Kara Lomen). Use Browse the claims to open any individual Bates-stamped claim.
